MEMO FROM THE HEALTH OFFICE

 

The end of the school year is quickly approaching. Your student may have medication in the health office for you to take home. We are not able to store medication over the summer. Please pick up the medication during the last week of school between 8:00am and 3:00pm.

For safety reasons we cannot send medications home with students. Please note that any medications that expired during the school year have been turned in to Health Services for disposal. Please check the expiration date on any medication prior to bringing it to school in September.

A DOCTOR’S WRITTEN ORDER is required by California State Health and Safety Code for EACH medication to be given during the school day including over the counter medication. A Doctor’s written order is good for one year.

For your convenience, please find attached a new “Administration of Medication”  form  along with an “ Authorization to Carry “ form (this form only required IF your student carries medication with them) to be completed by your student’s physician. All medication must be in its’ original, labeled container. Please send any over the counter medication in the smallest container available (i.e.: Advil, a small bottle of 20 pills rather than bottle of 100).
